Industries Case Manager
Must be available Monday – Friday, 7:00am – 3:30pm
Description of Position: Responsible for coordinating agency services that enable an individual with intellectual disabilities to receive a full range of appropriate services in a planned, coordinated, efficient and effective manner to promote the well-being of the individual. Responsible for locating, managing, coordinating, and monitoring all proposed services. Responsible for making recommendations for needed medical, social, educational, and informal community supports needed by eligible person within the day Services Program at TradeWinds. Responsible for oversight of funding resources to assure they are adequate for planned services proved by TradeWinds. Completion of weekly, monthly and annual paperwork to ensure TradeWinds remains in compliance.
Education/Certification Requirements:
Bachelor's Degree preferred
Experience Required:
Must have one year of experience working directly with persons with intellectual disabilities.
Prior experience in developing and implementing individual programs, program evaluation, and in working with a team is desired.
